Thats done by the module "Table of content"

Just enable module and add

add [break] or [break=my page name] in your text

Honestly, I am not sure about these cache settings,
but I guess you can solve it if you play a little with the settings.
The Geek has written the optimal cache-settings in the administration panel.

But if you have not made any page breaks in your text,
but only attached the TOC-module to your module-set,
the TOC-module will not show up in your article.

Yup, the cache system is only an optional enhancement for some modules (mainly the top and featured module). When in doubt - you can set it to not use the cache. Module sets by default have the toc module in them. Putting the break bbcode as henri mentioned should make the TOC block appear as long as it is in your module set.
